Here is how it works. If we think you would benefit, your GP arranges for an accredited pharmacist we work with to visit you at home. They will spend time going through all your medicines and devices with you, how and when you take them, how they are stored, whether anything is causing side effects, and anything you find confusing or hard to manage. Doing this at home, unhurried, often brings up the small practical things that never come up in a short appointment.
The pharmacist then reports back to your GP, and you and your doctor sit down together to work through it and put a clear medication plan in place, adjusting anything that needs it so your routine is simpler, safer, and easier to follow.
It is fully covered by Medicare, so there is no cost to you, and it can be done around once a year if it is helpful.
