When it's relevant

You take five or more regular medicines, have recently been in hospital, or are unsure what something is for.

Why it's worth a visit

The more medicines a person takes, the greater the chance of side effects and interactions, and of taking something that is no longer needed.

How your GP can help

Your GP refers you. A credentialed pharmacist visits you at home, goes through everything you take and reports to your GP, who then agrees any changes with you.

What does it cost?

The review is funded by the Australian Government for eligible patients, and the plan your GP prepares afterwards is bulk billed.

Will my medicines be changed?

Only with your agreement, after your GP has discussed the report with you.

What should I have ready?

Everything you take, including vitamins, creams, puffers and anything bought at the supermarket.
