Our credentialed pharmacist, Minh Tran, visits you at home, goes through everything you take and reports to your GP.
This may be appropriate if
- You take five or more regular medicines
- You have recently come home from hospital, or your medicines have changed
- You are unsure what something is for, or you suspect a side effect

Referral
Your GP refers you. It can be arranged at any appointment.
At home
The pharmacist visits at a time that suits you, for about an hour. Have everything out, including vitamins, creams, puffers and anything from the supermarket.
The report
A written report goes to your GP with any suggestions.
With your GP
You and your doctor agree what to keep, change or stop, and your medicines list is updated.

What does it cost?
The review is funded by the Australian Government for eligible patients with a GP referral. The medication management plan your GP prepares afterwards is bulk billed.
Will the pharmacist change my medicines?
No. They make recommendations. Any change is decided by you and your GP.
Can a carer or family member be there?
Yes, please. It helps.
