Health assessments

Once in your forties and every year from 75, it is worth stepping back.

A longer, unhurried appointment with a nurse and your GP that looks at the whole picture rather than one concern.

Who it may suit

This may be appropriate if

  • You are 45 to 49 and have a risk factor for a chronic condition, such as family history, smoking, high blood pressure or weight
  • You are 75 or older
  • You care for someone who is 75 or older and would like their health reviewed properly

What to expect
  1. With our nurse

    Measurements, medicines, vaccines, mood, memory, hearing, vision, sleep, activity, and how things are going at home.

  2. With your GP

    A review of everything gathered, an examination and any tests that are due.

  3. Your plan

    A short written summary of what matters most and what happens next.

  4. Next year

    For patients 75 and over, a reminder to return in 12 months.

How long should I allow?

About an hour in total.

Can a family member come?

Yes, and for older patients it often helps.

What should I bring?

All your medicines, including anything bought over the counter, your glasses and hearing aids, and any recent letters from specialists or hospitals.

Is there a Medicare rebate?

Both assessments are bulk billed for eligible patients: the 75+ assessment once every 12 months, and the 45 to 49 assessment once. See fees.
