When it's relevant

You have diabetes or pre-diabetes, or symptoms such as thirst, passing urine often, tiredness or slow-healing sores.

Why it's worth a visit

Diabetes can damage the eyes, kidneys, nerves and heart without symptoms. Regular checks find problems while they can still be prevented.

How your GP can help

Your GP and our nurses review your blood tests, blood pressure, feet and medicines, arrange eye and kidney checks, and build a care plan with you.

How often should I be reviewed?

Every three to six months for most people.

What is the annual cycle of care?

A set of checks done over each year, covering blood tests, blood pressure, feet, eyes and kidneys.

Can I see a dietitian or podiatrist?

Yes. With a care plan, eligible patients can receive Medicare rebates for a limited number of allied health visits.
