When it's relevant

You live with a condition that has lasted, or is likely to last, six months or more, such as diabetes, heart disease, arthritis, asthma or a long-term mental health condition.

Why it's worth a visit

Long-term conditions go better with a plan and regular reviews, not visits only when something goes wrong.

Your GP prepares a GP chronic condition management plan with you, setting out your goals, your care team and when you will be reviewed.

What does the plan give me?

A clear plan, regular reviews and, for eligible patients, Medicare rebates for a limited number of allied health visits each year.

Plans and reviews are bulk billed for eligible patients.

Do I need to be registered with MyMedicare?

If you are registered, your plan needs to be done at your registered practice. We can help you register.
